db manager fails to import simple CSV into spatialite

Description
I swear I successfully imported a large CSV file into a spatialite database via DB Manager recently. It's not working anymore.
Steps to reproduce:
1. Open DB manager
2. Open a spatialite database
3. Click on the Import layer/file toolbar button
4. Open the file selector and pick the attached dbmanager_test_import.csv
5. Click on the Update options button
6. Click on OK
The import process will fail with this error message: "Error 8 Loading of layer failed"
