The add delimited text file dialog should provide field for Z and M values

Description
When adding data from csv-like file using the "Delimited File" dialog, adding geometry columns only proposes fields for X and Y coordinates. It could be nice to have Z and M options too, now that we have more features handling these values.
I guess using a wkt field with z/m enabled geometry might already work (didn't try!)...

For the record, delimited text provider does support Z dimension and M values through a WKT field.
